STUDENT-ATHLETE EXIT INTERVIEW QUESTIONAIRE |

In order to improve the quality of our program, the Department of Athletics seeks constructive feedback from the athlete. This questionnaire provides us with information which may be useful to coaches and administrators in improving coaching techniques, behaviors, strategies or policies that impact the student-athlete.
We ask you to voluntarily put your name at the top of this questionnaire. Your responses to questions on this assessment will remain anonymous. Coaches will not see your individual assessment but will be given aggregate information on the responses from all the student-athletes in their respective sport. Your identify is important if the Director of Athletics or his designee needs to follow-up on this questionnaire with an interview. Again, responses to questions on this document will remain anonymous from the coach.
