Workers' Compensation

Workers' Compensation is a state regulated insurance program that pays medical bills and replaces some of the wages that an employee may lose as a result of being injured at work or who have a work related disease or illness. Any employee who sustains an injury on the job must notify their immediate supervisor. The employee must complete the workers compensation form on the same day that the injury occurred. First Report of Injury or Illness

The DWC Form 1 cannot be completed by the employee. The form must be completed in its entirety by the supervisor along with the employee and turned in to the Office of Human Resources Immediately. The employee will also need to complete The Request for Paid Leave form and return to the Office of Human Resources.

 On the Texas Department of Insurance, Division of Workers’ Compensation website: www.tdi.state.tx.us
  1. Select: Workers’ Comp
  2. Select: Information for Injured Workers
  3. Select: TXCOMP
  4. Select: Healthcare Provider System
  5. Put cursor on Main Menu; Select: Find Doctor
  6. Add your criteria, make sure you checked the Approved to Provide Treatment box
